FCB ONLINE MEMBERSHIP APPLICATION INSTRUCTIONS


Before you fill out the FCB Online Membership Application, here are a few helpful hints to make things easier:

For those of you that use a screen reader, please use the "CTRL" key to shut off the speaking as early as possible after the page has loaded. Then use the "TAB" key to navigate to the first edit box, which will be for "First Name." (If you use Window-Eyes, you may have to use the "SHIFT + TAB" keys to get to this edit box as you may be taken to the "Last Name" one first.) Once there, hit "ENTER" to either turn the screen reader's Forms Mode on or to shut off MSAA labels.

For entering Phone Number and Date of Birth, even though you can enter the numbers without any special characters to separate specific sections of the number, it is recommended that you enter them in the following ways:

Phone Number: XXX-XXX-XXXX (Area Code first then your 7-digit Number with dashes in between the area code and first 3 digits of the Phone Number and again between the first 3 and last 4 digits of the Number.)

Date of Birth: MM/DD/YY (For Date of Birth, if your month and day have only one digit, it is okay to not use a "0" in front of the number. The year can consist of just the last two numbers of it. In either case, separate the month, day and year with forward slashes.)

When you are asked to select the Chapter Name and Number, use the following list to become familiar with the various chapters/special affiliates and choose the one either closest to your area or that best suits your interests:

After you have filled out the Birth Date, make sure to either turn the Forms Mode of the screen reader off or turn the MSAA labels back on and use the down arrow to read the next lines of the application. These lines have information on printing out a copy of the completed form as well as where to send in membership dues if you are applying as a Member-At-Large.

The areas of the form that are the minimum required to send it in are First Name, Last Name, Home Phone and E-mail. These are marked with an asterisk (*) before the area title. If you do not fill these specific areas out, you will be taken to an error message that will instruct you to hit the "Back" button on the browser, usually "ALT + Left Arrow" on the keyboard, to fill out the form again and re-send it.

With a screen reader, you may occasionally come across a section or two of the Application that may read twice, like the combo box for "Are you a patron of the Talking Book Library?" If this happens, be prepared to use your "CTRL" key to quiet your screen reader and you should be able to fill out that section and continue on as you would normally. This only happens occasionally and may not always happen in the same section each time.
